This study found that rehydration of patients with an ischemic stroke and a BUN/Cr ratio of >= 15 was associated with a decreased risk of stroke in evolution. The rehydration group received a normal saline bolus of 300 to 500 cc then 40 to 80 ml/hr x 72H. The second group did not receive a normal saline bolus and IVF rate was much less day #1 and was 0 ml/hr days 2 and 3.

Comment: Traditionally, a BUN/Cr ratio of > 20 suggests dehydration or pre-renal disease; a ratio of 10 to 20 is considered normal or post-renal disease; a ratio of less than 10 suggests intra-renal disease (renal damage decreases BUN reabsorption). These authors used a cutoff of 15, and gently rehydrated patients with a BUN/Cr ratio above this level. http://bit.ly/1lzZOmh
