CHADS2 Score: give 1 point for each: congestive heart failure, hypertension, age > 75 and give 2 points for history of stroke or TIA. TREATMENT: CHADS2 SCORE = 0: aspirin 325 mg/d. SCORE 1-2: either aspirin or coumadin. SCORE > 2: coumadin to an INR level of 2 to 3. COMMENT: newer anticoagulants may be superior to coumadin in some patients.
